Chief Complaint:
14 yrs. old young girl came with the complaint of bedwetting since childhood. She never had got control over her urination since childhood and she would daily pass urine in bed 2-3 times in bed. This was very embarrassing for her and her family. Because of this

14 anos de idade jovem garota veio com queixa de enurese desde a infância. Ela nunca tinha obtido controle sobre sua micção desde a infância e ela iria passar diariamente urina na cama 2 - 3 vezes na cama. Isso foi muito embaraçoso para ela e sua família. Por causa disto.
On Examination :
Abdomen and pelvis – flat, no pain , no tenderness, normal on palpaition
No exame:
Abdômen e pelve-plana, sem dor, sem distensão, normal em palpação.
Past History :
Complaint of bedwetting since childhood. Never gain control over urination.
No history of any major illness.
História passada:
Queixa de enurese desde a infância. Nunca manteve controle sobre a micção.
Sem história de alguma doença importante.
Family History :
No history of major illness
História familiar:
Sem história de doença principal
Menstrual History :
Irregular, delayed since beginning. Dysmenorrhoea. Pain better by warm fomentation.
História menstrual:
Irregular, atrasada desde início. Dismenorréia. Dor melhor por aplicação de calor.(tipo cataplasma)
Drug History:
Not any except painkiller occasionally for dysmenorrhoea.
História de drogas:
Não relata exceto analgésico ocasionalmente para dismenorréia.
Mental State :
The patient was very much disturbed due to conflicts going on between her and her mother as her mother was very dominating kind and would always keep finding faults in her. Mother would criticise her in front of relatives and friends. The grandmother ( who is my next door neighbour ) also described her daughter to be very dominating and doing over parenting.
The grandmother also described about conflicts occurred between this girl's parents at the beginning of their marriage. Immediately after the mother came to know that the husband had spoken lies to her about his qualification and his earnings. She felt cheated and betrayed. She wanted to break the marriage but to the interruption by the families she unwillingly continued with this relationship and became mother of two children. Grandmother said that she could never build up a strong bond of mother-child relationship with her children.
Estado mental:
A paciente estava muito perturbada devido a conflitos acontecendo entre ela e sua mãe. Como sua mãe era muito dominandora e seria sempre, estava mantendo a posição de semrpe encontrar falhas nela. Mãe a critica na frente de parentes e amigos. A avó (que é vizinha ao lado) também descreveu sua filha como sendo muito dominadora e faz isso sobre seus filhos e parentes.
A avó também decreve sobre conflitos ocorridos entre os pais da menina no início de seu casamento, imediatamente após a mãe descobrir que o marido tinha falado mentiras a ela sobre a sua qualificação e seu salário. Ela se sentiu traída e enganada. Ela queria quebrar o casamento, mas por interferência das famílias, involuntariamente continuou com essa relação e se tornou mãe de dois filhos. Avó disse que ela pode nunca ter construído um forte vínculo de relação mãe-filho com seus filhos.
Observation :
When they came for the first consultation, the mother was criticising this girl like anything and this girl did not say anything but she looked to be very angry over her mother from her face.

Quando eles vieram para a primeira consulta, a mãe estava sempre criticando a menina por qualquer coisa e esta não dizia nada, mas parecia estar muito irritada com a mãe pela expressão de seu rosto.

Likes to cover. Baths with Luke warm water always. Need Fanning but covering is must.

Analysis:
Problem of bedwetting seems to be functional and secondly psychological in this case. Patient seems to be quite affected by mother’s behaviour and conflicts going on between mother and father.
Feeling of ' being let down ' by mother was very strongly coming up in this girl. She was keeping all her anger suppressed so effect of vexation was obvious there to affect her mental health. The paediatric studies and research shows psychological factors like any kind of fear or insecurity are likely to be one of the most possible causes in cases of bedwetting. The family and social environment in which the child is growing affects the child’s mind and plays major role in overall mental and physical growth and development of it.
Análise:
Problema da enurese parece ser funcional e psicológico neste caso. Paciente parece ser bastante afetado pelo comportamento e os conflitos entre mãe e o pai.
Sensação de estar decepcionada pela mãe foi chegando muito fortemente nesta menina. Ela estava mantendo toda sua raiva reprimida por efeito do vexame, obviamente era capaz de afetar sua saúde mental. de ser uma Os estudos pediátricos e investigação mostram fatores psicológicos como qualquer tipo de medo ou insegurança como possíveis causadores de casos de enurese. O ambiente familiar e social em que a criança está crescendo afeta a mente da criança e desempenha papel importante no crescimento total mental, físico e o desenvolvimento dela.
Miasmatic Approach :
The Fundamental Miasm behind all the psycho-somatic disorder is Psora. The emotional state of patient in this case affecting physical health highlights underlying effect of Psora as the dominant miasm and the strongest feature of this case.
The feeling of suppressed anger, vexation, are suggestive of progress of the psora to sycosis.

Evaluation :
This case was considered to be a psychosomatic disorder and patient’s mental state was decided to be focused in centre in whole management of the case. Being a functional and psycho-somatic case and also taking the young age of patient in to consideration, susceptibility seems to be very good and high in this case. Absence of characteristic physical generals except the craving for milk and chilly thermal state makes the case dependent totally on mental features.

Este caso foi considerado como um transtorno psicossomático e o estado mental da paciente foi decisivo para centrar-se todo gerenciamento do caso. Sendo um caso psico-somático e funcional e também tendo a tenra idade do paciente em consideração, a suscetibilidade parece ser o fator determinante nesse caso. Ausência de características físicas gerais, exceto a ânsia para o leite e o estado térmico frio torna o caso totalmente dependentes de características mentais.

Bedwetting as it is. But grandmother said that patient has become touchy and sensitive especially with her mother. Previously whenever mother used to say anything to her she would be angry or would ignore but, now she has started listening to what mother is saying to her and she feels bad about it and start crying but does not become angry as before.
Action : Continue S.L. for 15 days

• 20/03/07:
Enurese continua como estava. A avó disse que paciente tornou-se melindrosa e sensível especialmente com sua mãe. Anteriormente sempre que a mãe costumava dizer qualquer coisa para ela ficava estaria irritada ou seria ignorar, mas agora ela começou a ouvir o que mãe está dizendo para ela, mesmo sentindo-se mal sobre o assunto. Começa chorando mas fica irritada como antes.
Acção: Continuar S.L. para 15 dias
5/4/07:
After 1month: Bed wetting completely stopped. Grandmother noticed little bit improvement between the relationship between this girl and her mother. This girl started communicating nicely with her mother and started helping her in the kitchen.

Dysmenorrhoea was less in last menses.

Depois de 1 mês: molhar a cama completamente parado. Avó notou alguma melhora entre a relação da menina e sua mãe. A menina começou a se comunicar muito bem com sua mãe e começou a ajudá-la na cozinha.
Dismenorréia foi menor na última menstruação.

No bedwetting. Menses is irregular but painless. Anger is less.
This case was under observation till 2008; the girl never had relapse of complaint of bedwetting thereafter. And her menstrual cycles also became normal.
Nenhuma enurese. Menstruação é irregular, mas indolor. Raiva é menor.Este caso ficou sob observação até 2008. A menina nunca teve recaída da queixa de enurese depois disso. E seus ciclos menstruais também tornaram-se normais.
ALTA DA PACIENTE.
